1. Click here to complete a State of Florida Application for Replacement License Plate. Please fill out all highlighted areas, print out form, and sign to complete.
2. Click here to complete a Saint Leo University Affidavit.
3. Copy vehicle owner's drivers license and current vehicle registration.
